In India, the burden of non-communicable diseases is increasing. Clinical pharmacists have a responsibility in lowering this burden. Objective: The study aimed to investigate pharmacist interventions in minimizing drug-related problems in diabetes with co-existing hypertension. Methods: Prospective observational study. Results: Out of 1914 patients, 914 were male (47.8%), and the mean age was M= 53.85, SD= 11.54 years. 628 (32.8%) cases were having nine different types of suggestions provided by an intervening clinical pharmacist, which included substituting the drug in 247 (12.9%) cases. The overall cost of the treatment & length of stay in days when compared to the number of generic drugs prescribed was found significant. Poor compliance was discovered to be a major factor in our study. Conclusion: Clinical pharmacists have a crucial role in minimizing drug-related problems. Particularly, there should be a greater emphasis on patient counseling and patient follow-up.
